OACAS Training Requisition Form

To request an OACAS Course/Series/Trainer:
  1. Download the attached OACAS Training Requisition Form.
  2. Fill in the form. 
    1. If you already have an agency-based facilitator available, please indicate their name(s) in the "Trainer Name/Email" column.
    2. If you do not have an agency-based facilitator and would like to request an OACAS sessional facilitator instead, please indicate this clearly.
  3. Save the completed form to your computer.
  4. Go to "Contact myOACAS Support" and select Reason: Scheduling Training Courses.
  5. Use File Attachment to attach and submit your completed Training Requisition Form.
